Center for Diabetes Management
Diabetes Education makes a Difference.
Hillcrest’s Center for Diabetes Management is built on this belief. Recognizing that a one-size-fits-all approach doesn’t work, the Center’s team of highly-trained dieticians, nurses and specialists work with each patient to tailor a program that includes diet, exercise and medication to fit his or her lifestyle. Through group sessions and one-on-one support, patients learn the skills they need to take control of their diabetes and live healthier lives.

2 simple steps to schedule an appointment or a class
- A physician referral is required. Ask your physician to fax or mail a referral to the Center.
- After we receive your physician’s referral our staff will confirm your insurance benefits (Many insurance plans and Medicare cover these services for their members), then call you to schedule an appointment.
